MyChart Proxy Access

MyChart proxy access allows authorized individuals to log into their own personal MyChart account and securely connect to information regarding a patient under their care. Proxy access can be used to view health records for minor children and adult patients.

To view data through MyChart, individuals designated as MyChart proxies are not required to be patients of Mount Sinai Medical Center themselves nor do they need to have an active Mount Sinai Medical Center MyChart account. If the individual requesting proxy access does not have a Mount Sinai Medical Center MyChart account, they will be provided with a unique access code and MyChart activation instructions. Upon activating their MyChart account, the designated proxy will have access to the patient's MyChart account.

To request proxy access, please download and complete the appropriate MyChart Proxy Access Authorization form(s) and return it to your participating Mount Sinai Medical Center physician's office or to the Mount Sinai Medical Center Health Information Management (HIM) Department. The Health Information Management (HIM) Department is located at 4300 Alton Road Miami Beach, FL 33140.

Please note that the identity of the patient must be confirmed by your Mount Sinai Medical Center physician's office or the Mount Sinai Medical Center Health Information Management (HIM) Department prior to providing MyChart access to the proxy.

Adult Proxy Access

An adult patient, age 18 years or older, can grant permission to another adult to access his/her MyChart account. This requires completion of the Adult Proxy Access Authorization Form with signatures from both individuals. MyChart proxy access may be revoked by request at any time. To request revocation of this authorization, please send an e-mail to MyChartSupport@msmc.com and include a phone number and contact information. It is understood that any such revocation will not be effective until received by the MyChart Support Staff at Mount Sinai Medical Center.

Child Proxy Access

Adults with legal guardianship rights to a child (younger than 18 years old) may request proxy access to the child's MyChart account. Each adult must have their own MyChart account for accessing the child's MyChart account and complete the Child Proxy Access Authorization Form.

Once the child reaches age 18, the proxy access authorization will automatically expire and the adult will no longer have access to the child's MyChart account. At that time, an Adult Proxy Access Authorization Form must be completed with signatures from both individuals.
